Your membership includes unlimited, scheduled in-person, and telehealth visits with your clinician for all membership services. One membership fee grants access to all services!

Any ordered labs or prescribed medications are not included in this cost.

You can choose to have your labs billed to your insurance by our contracted lab, or pay for them directly at our very reasonable prices.

At this time we are not able to accept insurance payments for clinician visit. You can use your insurance to cover the cost of any labs we may order or for any medications we prescribe.

In the traditional fee for service model, patients often find themselves trying to determine if the symptoms changes or side effects they are experiencing are worth the cost of a visit with their clinician. We don’t think you should have to make that decision. A monthly membership allows you to budget for unlimited appointments. We feel with this model we are better able to optimize your treatments by seeing you more frequently if needed, to adjust medications, assess for side effects, and monitor progress without breaking your budget. The $960/yr cost of membership is less than many insurance deductibles.

Federal Poverty Guidelines

Yes, you can cancel your membership at anytime. If however, you decide to re-enroll within 12 months time, you will be subject to a $250 re-enrollment fee.
